ROENTGENENDOVASCULAR INTERVENTIONS ON THE ADRENALS IN PATIENTS WITH ARTERIAL HYPERTENSIONS THE CHOICE OF TECHNIQUES AND RESULTS

A study was made of the efficacy of different techniques of roentgenendovascular (REV) interventions on the adrenals in 251 patients suffering from arterial hypertension. REV hydrodestruction was employed in 14 patients. Over the first year observation period all operated patients developed recurrence of hypertension. REV acute occlusion of the venous bed of the adrenals by trombovar was provided to 31 patients. In the long-term period, good results persisted in 75% of patients. REV electrocoagulation of the central vein of the adrenal gland was performed in 148 patients. Good long-term results were noted in 45% of patients. Combined occlusion by trombovar and electrocoagulation was used in the treatment of 56 patients. Of these, good results were recorded in 88% of cases. The indication for electrocoagulation is adrenal hyperplasia without colleteral overflows revealed at phlebography. Otherwise, in the presence of collateral overflows there may occur the leakage of adrenal blood to the systems of the superior and inferior caval veins after electrocoagulation and occlusion of the central vein trunk. As a result, the efficacy of operation will appear temporary. In the presence of the developed superficial venous network of the hyperplasied adrenals and numerous collaterals to the systems of the caval veins, acute occlusion by trombovar coupled with electrocoagulation of the venous bed of the adrenals may be indicated to suppress adrenal hyperfunction.
