ASSESSMENT OF MAGNETIC RESONANCE ANGIOGRAPHY POTENCY FOR DIAGNOSIS OF CAROTID STENOSIS AND QUALITY CONTROL OF SURGICAL INTERVENTIONS

The aim of the study was to evaluate the potency of magnetic resonance angiography (MRA) for diagnosis of carotid stenosis.

Materials and methods: 28 patients with carotid stenoses and different grades of cerebrovascular insufficiency were investigated by ultrasonic duplex scanning (UDS), phase-contrast, 2D and 3D TOF MRA. MRA examination was combined with brain MRT, 19 patients were operated, 16 patients underwent postoperative ultrasonography and MRA. Results: Correlation of MRA and UDS results of stenosis grade and 18 distal plaques assessment revealed 100% sensitivity and specificity of MRA diagnosis.

Conclusion: MR angiography appeared to be a reliable diagnostic tool for assessment of pronounced atherosclerotic narrowing of carotid bifurcation and hemodynamic impact of internal carotid artery tortuosity at any level. Therefore it can be used as a screening method for patients suspected for atherosclerotic lesions of carotid arteries. Combination of MRA and UDS in majority of cases is sufficient for decision making of carotid endarterectomy and enables to avoid invasive radiological angiography. In postoperative period MRA can control the quality of surgical treatment.
