CONGENITAL VASCULAR MALFORMATIONS OF UPPER EXTREMITIES. DIAGNOSIS AND FURTHER INVESTIGATION WITH DUPLEX DOPPLER ULTRASOUND COMPARED WITH ANGIOGRAPHY

The paper presents the role of duplex Doppler ultrasound compared with angiography in the diagnosis of congenital vascular malformations (CVM) localized on upper extremities. Duplex Doppler ultrasound features of CVMs were described in B-mode images of malformated vessels and Doppler characteristics of blood flow-particularly flow resistance parameters, namely: resistance index (RI) and pulsatility index (PI) calculated from Doppler spectral waveforms obtained from arteries which feed CVMs. In 32 patients with CVM of upper extremity duplex Doppler examinations were able to cathegorize malformations into 3 groups: venous malformations in 12 patients, microfistulous a-v malformations in 5 patients and macrofistulous a-v malformations in 15 patients. RI and PI measurements were taken not only from upper extremity affected with CVM but also from the contralateral unaffected limb for comparison. Further duplex Doppler investigations were performed after therapy to assess its effects. Duplex Doppler findings were always consistent with clinical data and with angiography.
