CURRENT ASPECTS OF LUMBAR SYMPATHECTOMY FOR LOWER LIMB ARTERIOSCLEROSIS OBLITERANS

Lumbar sympathectomy for more then 70 years has been used for treatment of lower limb arteriosclerosis obliterans. However some aspects of surgical effect on microcirculation remain disputable. The impact of lumbar sympathectomy on blood flow in skin, muscles and bone matrix was assessed by laser Doppler flowmetry. It was shown that sympathectomy elicits activation of microcirculation in all tissue types dependent on the initial ischemic stage. Valsalva maneuver and nifedipine spasmolytic test are suggested as methods of preoperative prognosis for surgical outcome.
