A NEW SYSTEM FOR ENDOLUMINAL STENT GRAFTING OF THE AORTA.
A TECHNIQUE OF TWO SIMULTANEOUS ACCESSES

The aim of the present work was an attempt to refine the technology of endoluminal stent grafting of the thoracic and abdominal segments of the aorta by means of the design of a new system. It is based on the principle of gaining an additional access to the arterial bed via the main branches of the aortic arch. The theoretical development of the system carried out in 1994 was followed by Manufacture of an operating model. Subsequently the system was adapted to the use under clinical conditions. The superior access is obtained by the paracentesis of the axillary artery using the Seldinger technique. The design philosophy of the system allows to introduce an external sheath into the aorta by pulling up its cranial end via the superior access which considerably enhances its flexibility. The stent graft is attached directly to the holder at several sites, providing a fairly accurate positioning and fixation of all parts of the graft in the aortic lumen. The possibility of controlling the holder and the balloon catheter via the superior access prevents undesirable dislodgement of the graft by blood flow during its expansion and attachment to the aortic wall. The system may be recommended for urgent endoluminal stent grafting of the aorta. Further studies into the clinical use of the system are assumed.
