THE USE OF ENDOSCOPIC TECHNOLOGY FOR INTERRUPTION OF THE COMMUNICATING VEINS OF THE LEG IN CHRONIC VENOUS INSUFFICIENCY

The purpose of the work was to study a possibility of interruption of the communicating veins under the crural fascia by means ofendoscopic technology. Altogether 73 operations were performed in 65 patients with chronic venous insufficiency because of postthrombophlebitic syndrome and decompensated varicosity. The conventional Linton technique employed in such patients before produced a great number of complications in the form of suppurations, skin necroses, and lymphorrhea. The endoscopic technology using the Johnson & Johnson and Olympus systems for laparoscopic operations permits interruption under video control of the incompetent communicating veins, employing two trochars inserted via 1 cm incisions under the crural fascia. The results ofendoscopic operations are compared with those performed in keeping with the conventional Linton technique. Considerable advantages of the use ofendoscopic technology are marked. The hospital stay of the patients after operation could be reduced 2-fold whereas the incidence ofpostoperetive complications almost 3-fold. The results of the work allow the principal conclusion to be drawn that endoscopic interruption of the communicating veins in patients with chronic venous insufficiency of the lower extremities makes it possible to eliminate the need for the use of the conventional Linton technique.
