POSTINJECTION PYOINFLAMRAATORY AND TRAUMATIC VASCULAR LESIONS IN DRUG ADDICTS (STATISTICS, DIAGNOSIS, IMMUNE STATUS, SURGICAL POLICY, METHODS OF CONSERVATIVE THERAPY)

The review is an analysis of the reported, data on postinjection complications in drug addicts taking opium drugs and their homemade surrogates for injection to the large venous and arterial vessels. Current concepts of the character of cellular and humoral immunity derangement secondary to intravascular injection of narcotics are presented. Methods of the diagnosis of vascular complications in drug addicts are characterized. The author describes a broad range of postinjection traumatic, pyonecrotic and inflammatory complications that occur in the great vessels of the extremities, the types of surgical interventions and versions of surgical policy in patients with a false infected aneurysro of extremity arteries. It is pointed out that there are substantial discrepancies in the choice of surgical policy and that the results of surgical treatment of such extremity loss and life threatening complication are inconclusive, The causes of a wide variability of the course of extremity ischemia after forced ligation of the great arteries under conditions of the purulent process in the wound remain obscure.Current approaches to the treatment of surgical complications in drug addicts using efferent methods are discussed.
